Request for CASA Referrals may be completed and submitted only by dependents’ attorneys, county social workers and judicial hearing officers. Download the referral form here. Please contact the SFCASA Program Assistant at 415.399.6136 with questions regarding the submission of a referral or to follow up about the status of a referral if you are the referent. Only county social workers should submit their completed request for CASA referral forms to the CASA office by fax (415-398-8068) or regular mail (marked “Confidential,” c/o SFCASA, 100 Bush Street, Suite 650, San Francisco, CA 94104).
